• 帰国届 Notice of Return
• 修士課程　退学願／在学期間延長届
  Master's Program Request for Withdrawal/ Notice of Extension of Enrollment Period
• 博士課程　退学願／在学期間延長届
  Doctoral Program Request for Withdrawal/ Notice of Extension of Enrollment Period
• 改姓名届　Notification of Name Change
  Submit this notification both to the Graduate School Office and the respective Department Office.
• 学生証再交付願　Request for Re-Issue of Student ID Card
