Notification Forms (for Students in the Undergraduate School of Science)
- 休学願 Request for Leave of Absence
- 復学願 Request for Resumption of Studies
- 留学許可願（全学交換留学など）
Request for Approval to Study Abroad (through USTEP etc.)- 渡航日の2か月前までに、留学先の受入通知書を添付した上で学科事務室に提出してください。留学先の大学で修得予定の単位認定を希望する場合は、留学許可願提出前に学科事務室に相談してください。
- Submit the form attaching an acceptance letter to your department office at least two months prior to the date of travel. If you wish to transfer credits you will earn at your host university, contact your department office before submitting the form.
- 学術調査等のための海外渡航届（2ヶ月未満）
Notice of Overseas Travel for Academic Research (Less than 2 months)- 学術調査・学術実験を目的として渡航する学生は、渡航の2週間前までに入力してください。
- If you are traveling for the purpose of academic research or academic experimentation, fill out the Forms at least two weeks prior to your travel.
- 海外旅行届（私事渡航用）
Notice of Overseas Travel (only for Japanese students)- 留学・学術調査等以外の海外渡航をする学生は、渡航前に学科事務室へ提出してください。
- 留学生一時帰国・海外渡航届
Application for Temporary Leave and Overseas Travel for personal reasons- 学術調査等以外で一時帰国・私事渡航する外国人留学生は、渡航前に学科事務室へ提出してください。
- If you are going back to your home country temporarily or traveling abroad for personal reasons, submit the form to your department office prior to your trip.
- 帰国届 Notice of Return
- 海外に渡航した学生は、帰国後に学科事務室に提出してください。
- Submit the form to your department office upon your return to Japan.
- 退学願 Request for Withdrawal
- 改姓名届 Notification of Change in Name
- 学務課教務チーム（学部担当）窓口及び学科事務室にも届け出ること。
- Submit this notification both to the Student Affairs and the respective Department Office.
- 学生証再交付願 Request for Re-Issue of Student ID Card
- 学生証の再交付手続きは、専攻事務室ではなく、学務課教務チーム （学部担当）窓口で行ってください。また、紛失・盗難の場合は、まずユーティリティーカード管理室に電話（0120-240-751）にて連絡してください。
- If you wish to have your student ID card reissued, submit this form to the Undergraduate School Office and take the necessary procedures. In case your Student ID Card has been lost or stolen, first of all, call the Utility Card Office at 0120-240-751. This call is toll free.
